Please only submit 1 request per tournament. If the tournament is on your schedule your permit application has been submitted. OMHA typically approves permits no more than 2-3 weeks prior to the tournament start date. You will not receive your permit before this time.
All sections are required to be completed. Once completed and submitted your travel permit will be requested from OMHA. You will receive your permit once approved by OMHA. (This can take time, apply early) Please note there will be a $20 fee for any travel permits outside the OMHA.
